Arrive 30 minutes prior to your new patient visit and 15 minutes prior to you F/U appointment. Have your ID, auto insurance card and police report available. Update the office with your demographics and insurance information prior to your visit. 

Appointments may be scheduled by calling our office at 386-917-0007.

A $25 fee may be charge if patient does not show or cancels without a 24 hour notice. Please call our office to cancel any appointments, so that we are able to help you reschedule for a convenient time.

In the event of a severe situation, or one in which you are in doubt, go immediately to the Emergency Room of the nearest hospital.

Prescription refills should be requested during office hours. Please contact your pharmacy directly to send us an electronic refill request Allow 48-72 hours for all request over the phone or fax to be process. 

If you find it necessary to contact the doctor after normal office hours, please leave a message on the answering machine with your full name, telephone number and reason for the call.  The on call physician will return your call if it is urgent. Otherwise please call back during our regular office hours.

A fee may apply and it may take up to 1 week for processing.
